EPE Programs to Submit Forms

All SA 160.1 forms are due to be emailed or faxed to NYSED on February 16, 2012.  Submitting forms prior to February 16th may result in agency claims which do not reflect data that will be frozen in ASISTS on February 15, 2012.  Therefore, forms should not be printed or submitted before February 16th.

The form submitted on February 16, may be submitted without signatures; however, the agency will have 10 business days (March 1, 2012) to submit to NYSED the original signed copy of the SA160.2.  NYSED cannot guarantee EPE reimbursement to programs that fail to submit claims on February 16, 2012 or that fail to submit an original signed copy by March 1.
